The mission of the Arkansas Department of Health (ADH) is to protect and improve the health and well-being of all Arkansans, including its’ workforce. ADH is committed to open and honest communication. By completing an ADH Exit Survey at the conclusion of employment, employees and former employees assist Executive Management with identifying best practices as well as needed improvements. Survey question responses through this portal are completely anonymous. If preferred, a hardcopy Exit Survey can be requested from the immediate supervisor or downloaded through the ADH Intranet site before an employee’s last day of work. Hard copy Exit Surveys can be mailed free of charge with a postage paid envelope provided by ADH, but only one submission process (electronic or hard copy) should be used.  Thank you for taking the time to respond to the Exit Survey.
